Job Description

• Perform telephonic or virtual triage for hospice patients, caregivers, and family members, assessing symptoms and determining appropriate interventions.
• Evaluate and prioritize patient concerns.
• Provide clinical guidance and education to caregivers on symptom management, medication administration, and comfort measures in accordance with hospice protocols.
• Determine when issues can be managed remotely versus when escalation is required.
• Accurately document assessments, interventions, and recommendations in US-based EMR/EHR systems in a timely manner.
• Collaborate with interdisciplinary hospice teams (physicians, RNs, social workers, chaplains, and care coordinators) to ensure continuity of care.
• Follow US hospice regulations, HIPAA guidelines, and organizational policies.
