Level of assistance: One support worker Equipment in use: Foldable wheelchair Mobility / Transfers: Alan requires one or two-person assist for transfers; may require assistance getting in and out of vehicle Special instructions: - Vehicle must have sufficient space to store a foldable wheelchair -
