About the Role
Your Opportunity
The Mission Australia Supported Housing program in Katherine provides tenancy support to clients who are facing homelessness, tenancy risks and other related issues. The Case Manager will support to individuals and families who are at risk of homelessness and having difficulty maintaining their tenancy.
Your key responsibilities will be to:
- Respond to referrals of clients to the service from internal and external support services
- Undertake initial registrations for clients, and organise introductions to the program
- Facilitate and develop relationships with organisations or services who work with TSP clients
- Create individualised case management files for all clients
- Complete a range of internal and external reports
