The Senior Coordinator, CHW Programs supports the delivery of the Transportation and Gathering Centre programs. The role actively contributes to planning activities for Elders, address social isolation, coordinate with other programs and partners to support Elders’ goal of living well and safely in their home, as well as identify any service gaps. The role also supports the program delivery by recommending improvements.
