About the Role
The Support Coordination Team exists to connect participants with an NDIS plan to supports and services. The team is the first point of contact for NDIS participants, families and Support Coordinators who are seeking information about New Horizons or who have chosen us to provide services for them under an NDIS plan.
We empower our customers by supporting them to build independence, identify and achieve goals, engage in activities of daily life, and encourage genuine connections within the community.
