Overview

Deliver data-driven insights to inform and strengthen population health programs, improve patient outcomes, and advance Population Health initiatives. This role collaborates across quality, care management, and clinical teams to support strategic decision-making and optimize program performance through actionable analytics. Works under the supervision of the Enterprise Population Health Program Manager.
