Patient Coordinator

Responsibilities

• Act as the first point of contact for all patients, answer patient calls, emails and questions, including finding insurance estimates.
• Educate patients and their families on the care plan, provide details related to treatments, procedures, medications, and continuing care requirements.
• Discuss cost of service, insurance coverage, and payment options with the patient.
• Serve as a liaison between patients and healthcare providers.
• Assist care team with setting goals for quality assurance and best practices.
• Develop and maintain a clear and up-to-date knowledge of the services available, patient pathways and availability to ensure effective and efficient utilization of appropriate service provision by directing patients to these services as appropriate.
