Our Community Mental Health Teams (CMHTs) are dedicated to providing planned and effective mental health support and interventions tailored to the individual needs and goals of each service user. Care is delivered in partnership with service users, their loved ones, and other services to ensure a holistic and person-centred approach.
- Community Mental Health Teams (CMHTs) work closely with other agencies and organisations to promote the health and wellbeing of the local population.
- Responsibility for care is shared between the CMHT, partner organisations, the service user, and their loved ones.
