Responsibilities:

• Review and analyze HHC medical records for accurate ICD-10-CM and HCC code assignment
• Ensure documentation supports risk adjustment coding in compliance with CMS and payer guidelines
• Collaborate with clinicians to clarify diagnoses and improve documentation accuracy
• Maintain productivity and quality standards while meeting strict compliance and audit requirements
• Stay updated with current coding regulations and best practices
