Experienced RN Case Manager Needed

Join our team as an RN Case Manager, available in various formats including on-site, hybrid, or remote in Cambridge, Ontario. We offer both contract and permanent positions.
As an RN Case Manager, you'll play a vital role in patient care. You'll monitor hospitalized patients, update their records, and work closely with medical teams to coordinate services. Strong communication and assessment skills are essential as you handle approvals for procedures and liaise directly with patients regarding their care and benefits.
Key Responsibilities:
• Conduct daily monitoring of hospitalized patients
• Obtain assessments from medical consultants

Requirements:
• Minimum 2 years as an RN or Case Manager
