About the Role
The Hospital Discharge Team are based at the Diana Princessof Wales Hospital. The HospitalDischarge Team is responsible for the safe and effective discharge of patientsfrom hospital into the community and also the prevention of admission tohospital. The Hospital Discharge Teamworks in partnership with the Acute Trust ensuring effective prevention anddischarge pathways are followed. Thepostholder will provide professional support to those staff working in theHospital Discharge Team. The team operates 7 days per week working between thehours of 8.00am and 8.00pm.
