The role of Support Worker is within a multidisciplinary team (MDT), supporting individuals to regain independence and achieve rehabilitation goals in community environments, including working alongside the community nursing team to provide treatment in patients’ homes. The role requires a compassionate, patient individual who can communicate well, work independently (including lone working) and as part of the wider MDT.

A basic understanding of therapy principles, rehabilitation techniques and nursing skills – observations, phlebotomy, would care and insulin administration – is required. Training will be provided if necessary. Relevant experience in community se...
