Lead Community Health Services in the Erie St. Clair region as a full-time Personal Support Manager at CarePartners. Oversee care coordination and support to improve patient experiences.
As part of CarePartners' leadership team, you will coordinate and supervise Community Support Workers while developing effective Care Plans. This full-time role requires travel across Sarnia-Lambton, Windsor-Essex, and Chatham-Kent counties, with hours from Monday to Friday. Weekend and evening on-call participation is necessary, enhancing patient care and mentoring healthcare teams.
Key Responsibilities:
