This full-time role within the Urban Health and Homelessness Services department focuses on delivering direct support to individuals facing complex challenges. As a Case Manager, you will utilize harm reduction and trauma-informed methods to assist clients in navigating health, legal, and financial services. You will build partnerships with other organizations to streamline access to necessary resources and support for those living in shelters and encampments.
