As a Care Partner, you’ll guide customers through their care journey with compassion. Working closely with families, clinical teams, and external providers, you’ll develop holistic health plans, coordinate services, and ensure customers receive safe, person-centred care that enables them to age well at home or in a place of their choosing.
This is a full-time role and can be based at our either our Queanbeyan or Cooma NSW branch.
This role involves occasional travel across the ACT and Cooma regions, including some regional and remote locations such as Jindabyne, Dalgety, Crackenback and Berridale.
