About the Role
Worthing Medical Group is a large,forward-thinking primary care practice delivering a well-established, highlyproactive frailty service across care homes and for housebound patients. Wecurrently support over 900 patients living in care homes, with a whole-systemmodel designed to deliver high-quality, coordinated care to one of our mostvulnerable patient groups.
The service is clinically led by GPLead Dr Anna Robertson, working alongside a team of three highly skilledAdvanced Nurse Practitioners, supported by dedicated care coordinators. Thiscreates a strong, collaborative and well-supported environment, enablingclinicians to focus on delivering high-quality care.
We operate a structured, proactiveapproach including care home ward rounds, case management and multidisciplinaryworking, ensuring patients receive the right care at the right time.
